Well, some people are casual gamers, some people are '|-|4]2|)|<0]23' and some people have addictive personlities.
I personally think that it's more to do with how much time you chuck in than anything else...If you're playing MMORPGs so much in a day that you don't have time to attend to things in your real life properly...then things start to slip...the worse your life gets, the more you need your virtual one for your fun/escapism...and it becomes a staple, until eventually you're more interested in the game because people understand you there, it offers rewards, respect, and whatever else you may be missing outside of it...Vicuous circles and snowballs 
So many people compare MMORPGs with drugs, but I don't think that makes retailers or game makers dealers...because it's only really harmful for a minority of people and there could be any number of factors effecting what your play time is, how much you're putting into it emotionally, and how much you're getting out, and it's not a chemical addiction like drugs...it's mental, so more abstract and harder to define. Sometimes I can get really into my games...and sometimes I can get really into my books The book doesn't talk back/offer interaction, so there's no way I could misconstrue it as a 'life', but if it was I dunno, 20 thousand pages long and I sat there reading it non stop and died, my ghost would be miffed and surprised if my loved ones tried to sue the author.
I think forgetting about life enough to neglect sleep, food and drink to the point where it's fatal is somewhat extreme and only happens to a tiny minority of people...But that said, it's easy to do. How many of you have missed a nights sleep due to a fleet op or something 
It's not that much of a stretch of the imagination from there.
edit: forgot to add that one of those factors is probably being isolated. If you were living with someone they'd probably make sure you were drinking and after seeing you glued to the computer screen for so many hours straight would kick your ass to bed.
